If you have lost or misplaced your name badge you will be required to pay $8.00 (exact cash only) for a new badge.
If you currently have your badge and it is not working, then bring it to the Office of Human Resources for a replacement badge.
Employee Name Badges are processed through the Name Badge Request Form, Monday through Friday, 8:00am- 4:30pm.

In order to change your name, The State of Texas requires that an individual provides a certified copy of a Social Security card in order to change their name. Please click on the link below to update your name.

Please follow the steps listed below:
STEP 1: Download, complete, sign, and save the PDF fillable Change Address Form.
STEP 2: Click HERE to submit the completed form to HR.
STEP 3: The TRS Change of Address Form must be sent to TRS via mail.
